Thesis Title: Association Between Early Marriage and Its Impact on Womens Health : A Study From Rohingya Refugee Context
Overview: Background: Early marriage, women's poor reproductive health and child well-being are important areas of concern in most developing countries. Previous research suggests that child marriage may be accelerated during times of crisis and insecurity as resources are scarce and child marriage may be a survival strategy for girls and their families. This study attempts to identify the relationship of early marriage that increased exponentially and how its impact on Rohingya women’s health living in Cox’s Bazar, Bangladesh. Methodology: It was a cross-sectional study. The study adopted a multi-stage sampling where the study area was clustered in numbers and one cluster (namely camp-7) was selected randomly. A total 416 women were selected as sample based on their availability on interview date and hence interviewed following a structured questionnaire to collect socio- demographic, early marriage and health related information. Reproductive health index (RHI) was calculated as indicator of women’s health and later compared with women’s status. A qualitative focus group study was also conducted to focus the early marriage experience and how it might have affected their health and to their life chances, Results: The study revealed that 45.2% women were married before the age of 18 years and among them 14.9% married before <14 years unfortunately. The use of contraception was very low (around 9%) and around 23% women had child before the age of 18. Almost half of the women faced pregnancy complications, miscarriage happened for more than 16% of women, the recommend number of antenatal care visits during pregnancy was very low and almost all the deliveries were taken place at home without the help of professional doctors. The 58.1% have average RHI those who got married at 14 and the RHI score was high for women who were married at age 18 and RHI score were mostly average or poor when married before 18 years. Women having poor RHI were 7.46 times (OR 7.460; 95% CI: 4.64- 11.98) more likely to have pregnancy complication and 3.381 times (OR 3.381; 95% CI:1.96- 5.82) more likely to experience miscarriage than women who had high RHI score. The results show that an early age at marriage had detrimental effects on the reproductive health status of women. Women married at an early age were exposed to frequent childbearing, unplanned motherhood and abortions, which negatively affected their nutritional status. Qualitative findings reveal that women were forced to marriage because of the uncertain future they were in and families deemed marriage might help their cause to safety and security. Conclusion: Program should be focused on delay of the entry of adolescents into wedlock and motherhood through information, education and communication. More emphasis needs to be put on meeting the reproductive needs of poor adolescent mothers and improving the nutritional status of their children, to break the vicious circle of poor reproductive health and poverty.
